Using a saved template (.crtx) to build your chart does not usually bring the Chart and axes titles.

Consider the following chart created using a chart template (.crtx file) for the sample data as shown:
To modify the chart title, click inside the title and use the formula bar.

Optionally, if you want the chart title to be dynamic, enter the title in a particular cell, and link the title to that cell using a "=" as shown:
To add the axis titles, click on the chart and then click on the "+" icon that appears at the top of the chart.
Here, the Axis Title check box would stay checked, but no axis title would be visible:
Uncheck and check it again:
Add suitable titles and your chart is ready!
